What is Student Activities?
The Department of Student Activities acts as an advisor,
ally, administrator, educator and mentor to the students of Southwestern
University by facilitating learning experiences through leadership
opportunities, student organizations, social activities, and other services that promote personal
growth and the empowerment of students for success in our diverse community, as
well as beyond.

Voice Mail Request Form (HTML)
Application for Student Organization Emergency Funds (Adobe PDF) - This form is used to request funds for a specific event/project that was not included in the organization's current budget allocation
Application for Student Organization Funds (2004-05) (Adobe PDF) - This form is turned each April to request funding for the upcoming academic year
Community Chest Funding Application (Adobe PDF) - The Community Chest Committee funds "large scale programs" that are inclusive and fun. More details are included on the application itself.
Signature Card (2004-05) (Adobe PDF) - This form is required of all active organizations and lists current officers and advisor
Traffic Violation Appeal Form (Adobe PDF) - This form must be filed within 5 days and be accompanied by a copy of the ticket
